Urgent Appointments
To request an urgent appointment for today (Monday to Friday):
- Phone us on 0121 743 2626, Monday to Friday from 8 am
- Visit the surgery and speak with a receptionist, Monday to Friday from 8 am to 6 :30pm
When you get in touch, we’ll ask what you need help with. We will use the information you give us to choose the most suitable doctor, nurse or health professional to help you.
If your appointment is not urgent you will be asked to submit the online consultation form.
Routine Appointments
To request routine nurse / chronic disease appointments in the next 4 weeks:
- Phone us on 0121 7432 626, Monday to Friday from 8 am
- Visit the surgery and speak with a receptionist, Monday to Friday from 8 am to 6:30 pm
- Submit online triage request for non‑urgent medical and admin queries
When you get in touch, we’ll ask what you need help with. We will use the information you provide to choose the most suitable clinician to assist.
Sick (Fit) Notes
If you’ve been off sick for more than 7 days:
- Fill out a sick note request form (via AccuRx).
- Phone or visit the surgery from 10 am to 6 pm
If you’ve been off work sick for 7 days or less:
Your employer should not ask for medical evidence; they may ask you to confirm your illness by filling out a form yourself when you return to work (self‑certification).

Test Results
Getting your test results:
- If your results show you need more tests or treatment, we will contact you
- Once a doctor has reviewed your results, you can view them:
– in your NHS account or via the NHS App
– Submit an online admin request (via AccuRx)
– Phone or visit us between 10 am to 4 pm to receive them

If you need help with your appointment
Please tell us:
- if there’s a specific clinician (doctor, nurse, etc.) you prefer
- if you prefer phone, face‑to‑face, video call, text, or email consultation
- if you need an interpreter
- if you have any access or communication needs
Chaperone Service
- All patients may request a chaperone for any consultation, examination or procedure. This can be a family member or friend, or, if you prefer, a trained member of staff.
- Ideally request this at the time of booking so arrangements can be made. If requested later, we will endeavour to provide a formal chaperone or may need to reschedule your appointment.
- In some cases, the clinician may require a chaperone, in line with our chaperone policy.
Teenage Friendly – Can I See the GP or Nurse on My Own?
- We aim to make services teenage‑friendly and maintain confidentiality, especially for ages 12‑16
- Teenagers can see a clinician alone if they wish
- If a parent/carer accompanies them for part of the visit, the young person may still be seen alone for part of the consultation
- People aged 16 or over can consent to their own treatment unless they clearly lack capacity
- Children under 16 can consent if they are considered “Gillick competent”
You have the right to accept or refuse treatment. If you lack capacity, consent must be from someone legally able to act or in your best interest.
You will be involved in treatment decisions, given information to help choose the right treatment, often through discussion with a clinician.
Cancelling or Changing an Appointment
To cancel your appointment:
- Reply CANCEL to your appointment reminder text
- Use your NHS account
- Phone us: 0121 743 2626 , Monday to Friday 8 am to 6:30 pm
Home Visits
If you believe you need a doctor to visit you at home (because you are housebound or too ill to attend):
- Telephone 0121 743 2626, preferably before 10 am
- Provide as much information as you can to the receptionist so the doctor can assess the urgency
- Calls are handled confidentially
Our Extended Clinical Team
You may not always need to see a GP—our team offers services such as:
- Nurse clinics
- Social Prescriber
- Mental Health Nurse
- Pharmacist (for medication-related queries)
- Physiotherapist
These services may better meet your needs than a GP appointment in some cases.
Non‑urgent Advice: If We Are Closed
- Use NHS 111 online or call 111
- For children under 5, call 999 if it’s serious
- Call 999 in a medical or mental health emergency (when life is at risk)
Note: All calls are recorded and kept for 36 months. Recordings are stored securely with restricted access.
